Linking international agricultural research knowledge with action for sustainable poverty alleviation: What works?

This report, published by the Center for International Development at Harvard University and the International Livestock Research Institute in Kenya, asks which institutions and approaches are most effective in using scientific knowledge to inform development strategies.

The authors evaluate five agricultural projects in Africa and Asia and suggest that the best strategies for closing the gaps between knowledge and action include: 'boundary-spanning' efforts such as creating partnerships to improve dialogue between researchers and local communities; 'use-driven' research directed at solving particular problems for particular groups of people; engaging stakeholders from the outset of a project; providing incentives for risk-taking; and improving project management and communication skills.

HIV drug policies and South markets: settling controversies

This paper proposes a model to provide better access to fairly priced antiretroviral (ARV) drugs for HIV-infected people in poor countries, while also safeguarding the interests of ARV manufacturers.

The authors explain what governments and brand and generic companies are doing to increase the availability of ARVs in developing countries, taking examples from Brazil, Canada, China, India, the United States and Thailand. They also discuss the implications of creating more South–South partnerships to produce and market ARVs; and the impact that the UNTAID–Clinton Foundation coalition has had on lowering ARV prices in developing countries.

The authors recommend an incentive-based strategy that includes international donors bulk-purchasing generic ARVs, individual governments providing financial relief packages for generic companies, and the WHO brokering negotiations between brand and generic companies.

Is Bayh-Dole good for developing countries? Lessons from the US experience

This article examines the US Bayh–Dole (BD) Act — a 1980s measure that sought to stimulate science-based economic growth by encouraging universities to patent inventions resulting from government-funded research — and assesses its suitability for developing countries.

The authors look at how and why advocates of BD-type initiatives sometimes overstate its impact in the United States and discuss the problems the act has caused for American biotechnology and information technology.

They outline the policy options for developing countries seeking to improve the contributions universities make to economic development and provide a list of safeguards that should be put in place before adopting laws styled after the act. These include no exclusive licensing, transparent patenting and government authority to issue additional licenses.

The authors conclude that policies to develop public sector research and development are context-specific and it is unclear whether any of the positive impacts of BD in the United States would arise in developing countries following similar legislation.

A world of science in the developing world

This collection of features and commentaries, published by Nature, reflects the broad spectrum of activities and opinions of members and associates of TWAS, the academy of sciences for the developing world.

With more than three dozen articles written by prominent scientists working on research or policy issues in the South, the collection examines a range of topics in science-based international development — from the relevance of subjects like mathematics or physics, to the increasing roles of biotechnology and renewable energy.

The achievements made and challenges still facing developing countries in key areas like agriculture, health, climate change and energy are also discussed. And evidence from across the South is presented to show how strengthening science can help achieve economic goals and what more is needed to ensure that knowledge and development are shared by all.

Health system reform in China

This series of commentaries and research articles — published by The Lancet, the Peking University Health Sciences Centre and the China Medical Board — addresses China's major health challenges, strategies and future. It has been produced by a group of 63 scientists from 10 countries with Chinese scientists making up two-thirds of the authors.

The research papers give scientific evidence on key health issues including the emergence and control of both infectious and chronic non-infectious diseases in China as well as the performance of China's healthcare system.

Authors of the series' commentaries further discuss a range of topical issues affecting China's health system, including the state of biomedical science and technology (see 'Progress in Chinese biomedicine a massive challenge'), medical research ethics, the lessons learnt from China's schistosomiasis control programme and the challenges the country faces in controlling HIV/AIDS.

The Millennium Development Goals Report 2008

This report summarises progress towards each of the eight Millennium Development Goals (MDGs) in ten regions, based on data available as of June 2008. Each goal is reviewed, clearly identifying the different targets embedded in each and presenting data to show progress to date and gaps in achievement.

The report emphasises the deep commitment made by the international community to meeting the goals and highlights some of the advances made in areas such as primary school education, health, access to safe drinking water, use of ozone-depleting substances and access to critical drugs.

At the same time, the report points to areas where the goals are unlikely to be met unless additional, or corrective, action is taken. These include child malnutrition, gender parity, sanitation, carbon emissions and international trade negotiations.

A key message is the need to improve developing countries' capacity to produce, analyse and disseminate data. The report points to a number of initiatives aimed at helping this happen.

Health and environment: Managing the linkages for sustainable development

This report highlights the key findings of the Health and Environment Linkages Initiative, set up by the WHO and UN Environment Programme to identify ways of integrating environment and health considerations into decision-making. The report outlines the ways in which health and environment linkages are usually defined and framed by policymakers, and describes the most common institutional and political barriers they face.

A review of formal impact assessment tools is given, along with a 'menu of options' for good practice application of impact assessment. The report highlights the importance of measuring the impacts of decisions made in terms that can be understood by policymakers. And it offers guidance on how to combine health and environment issues with economic considerations, describing real-world experiences from Jordan, Thailand and Uganda.

Meeting the challenge of antibiotic resistance

This article examines the rising rate of antibiotic resistance, and proposes action to reverse the trend. The authors call for leadership at international and national levels, a change in the behaviour of consumers and providers, and the development of antibacterial agents to match existing public health needs.

They suggest that international bodies such as the WHO should provide coordination and resources for generating key information on the burden of antibiotic resistance, and call for individual governments to set up multidisciplinary action programmes. They also examine the methods needed to better inform the public and discuss alternative approaches to developing new antibacterials, including product development partnerships and gap analyses.

Clean Energy Investment

This report examines ways to increase flows of domestic and foreign investment into clean energy infrastructure and technology in developing countries. It is a synthesis report of the International Institute for Sustainable Development's Clean Energy Investment project.

The authors outline the investment climate in developing countries and suggest ways that policymakers can remove barriers and establish incentives. They suggest there is a need for analytical national studies that highlight the obstacles to clean energy investment — such as a lack of clear guidance on future energy policy, monopoly structures for existing producers, and weak environmental regulation — and a concerted effort at implementing policies to overcome these. The authors also discuss existing international investment law, suggesting ways in which this might foster more clean energy investment.

The state of food and agriculture 2008 — Biofuels: prospects, risks and opportunities

This report, published by the Food and Agriculture Organization, outlines the current state of the biofuels debate and examines the policies being implemented to support biofuels and those needed to address issues affecting the environment, food security and the poor.

The report begins with a technical overview of the types of biofuels available and then focuses on the economic and policy drivers of liquid biofuels and the short- and long-term prospects of biofuel markets. The environmental impacts of biofuels, together with the impacts on poverty and food security are examined and the policy challenges discussed, including proposals for a better policy framework.

The report suggests that rapid increases in demand for biofuel feedstocks pose an immediate threat to the food security of the urban and rural poor. But it also implies that in the long term, more demand for agricultural commodities may present opportunities for agricultural and rural development. The impact of biofuels on greenhouse gas emissions is found to vary according to location and agricultural practices, and the authors call for more harmonised approaches to assessing the impact of biofuel production on climate change.

Global environmental change research: empowering developing countries

This paper discusses ways of reconciling the Millennium Development Goals with environmental sustainability. Using an example from Brazil — the Large Scale Biosphere-Atmosphere Experiment in Amazonia (LBA) — the authors examine why researchers aiming to produce sound scientific understanding to support sustainable development often fall short.

Translating knowledge created by the LBA project into public policies proved difficult. The authors blame this failure on resource limitations, weak institutions and scientists' inexperience in applying science to real situations.

They suggest that establishing centres of excellence in developing regions is a necessary first step to creating a bottom-up approach to sustainability that includes innovative ways of assessing ecosystem services. These centres must be able to effectively use and produce applications-directed research and bring it to bear on decision-making related to environmental change and sustainable management of natural resources, say the authors.

Sea level changes along the Indian coast: Observations and projections

The authors of this article analysed simulation results from a regional climate model for the northern Indian Ocean to predict likely changes in the strength and frequency of tropical cyclones in the Bay of Bengal from 2041–2060.

They find that rising concentrations of greenhouse gases will lead to more frequent cyclones in the region, particularly during the post-monsoon period. In addition, the number of intense cyclones and storm surges will increase. These results are consistent with other trend analyses that show intensification of cyclones in the bay during the last century.

But the research described in this paper only deals with simulations from one future climate scenario. To obtain better regional climate projections, the authors suggest it is necessary to examine simulations from more scenarios.

Climate change risk: An adaptation and mitigation agenda for Indian cities

This paper, prepared for the Global Urban Summit, proposes a framework for building climate change adaptation and mitigation measures into India's urban renewal programmes. This involves multiple government stakeholders at national, state and city levels.

The author begins with a description of the urbanisation trends in India and outlines the climate risks facing the subcontinent, including changing rainfall patterns and the potential for more drought, flooding and extreme weather events like cyclones.

The author outlines the vulnerability of urban populations and suggests that reducing it requires a public policy shift towards mainstreaming climate change risk assessment, adaptation and mitigation measures into ongoing national hazard mitigation programmes, and linking them to urban renewal in the largest cities.

Specific measures highlighted by the author include making building data public, building flood defences, strengthening existing infrastructure to withstand disasters and relocating highly vulnerable populations.

The increasing intensity of the strongest tropical cyclones

The authors of this article use satellite data to examine trends in the maximum intensities that cyclones can achieve during their lifetimes.

Results from previous analyses of tropical cyclone trends have been questioned due to a lack of consensus regarding data reliability. Moreover results have not been matched to theory because the focus has mainly been on changes in mean tropical cyclone statistics.

In this article, the authors conclusively show significant increases in the maximum wind speeds achieved by the strongest cyclones across all ocean basins except the South Pacific Ocean, with the largest increases occurring over the North Atlantic and northern Indian Oceans.

These findings are consistent with the idea that as seas warm, cyclones become more intense because the ocean has more energy that can be converted to tropical cyclone wind.

ORCHID: Piloting Climate Risk Screening in DFID Bangladesh, Summary Research Report

This report by the Institute for Development Studies details the results of a pilot project in Bangladesh aimed at developing a screening process for the UK Department for International Development (DFID) to identify and manage climate change impacts on development investments.

The authors highlight predictions that climate change in Bangladesh may lead to stronger cyclones, increased flooding during the monsoon rains and exacerbated drought in the dry season.

They suggest that raising roads and improving drainage could be a cost-efficient way to reduce the impact. Other options recommended for managing risks include paying greater attention to infrastructure design in health, education and private sector development programmes; and to non-structural measures such as livelihood diversification, education and training about disaster risks and adaptation, and improved research and monitoring.

The authors conclude that DFID should support dialogue on disaster risk reduction and climate change adaptation, integrate priorities identified by the Bangladeshi government, increase emphasis for assistance on urban areas, and stimulate a multi-donor dialogue about water issues.

Tropical cyclones in a warmer climate

This paper discusses likely future changes in tropical cyclones, questioning whether they will become more intense following higher sea surface temperatures. The author outlines the different approaches currently taken to climate modelling and discusses the results of characterising current and future climate using the Max Planck Institute for Meteorology in Hamburg model, comparing them to observations.

Most climate models predict stronger tropical cyclones in a warmer climate, as an increase in latent heat provides more energy for the storms. But the author claims there is less evidence for a reduction in the frequency of storms in a warmer climate. Still, such a reduction could result from a general weakening of large-scale atmospheric circulation (which reduces the number of cyclones) caused by the rapid increase in water vapour that would follow a rise in global temperatures.

Expanding priorities — confronting chronic disease in countries with low income

Cardiovascular disease accounts for 30% of deaths worldwide and 10% of all years of healthy life lost to disease, and the figures are nearly as high in developing countries — 27 per cent and 9 per cent respectively. This compares with 10% of lives lost worldwide from HIV/AIDS, TB and malaria put together (12% in developing countries). So why have donors not invested as heavily into tackling non-communicable chronic diseases as they have with infectious ones? The authors of this article suggest several reasons: infectious diseases are in some ways easier to solve by a vaccine or drugs so it might seem sensible to use precious funding this way; Western donors may want to see epidemics contained quickly to avoid global spread; pictures of small African children dying of AIDS are more heartrending than a middle-aged man with hypertension, even if that man is supporting a large family; there is a myth that chronic diseases are more costly to prevent than infectious ones. This last issue is one that should be tackled strongly to spread awareness that low-cost methods can have an enormous effect on chronic diseases.

Therapeutic vaccination for chronic diseases: a new class of drugs in sight

Vaccination for infectious diseases is a vital method of prophylaxis, and has transformed modern medicine. By contrast, research into vaccines against chronic diseases has been less successful, in part because of the increased complexity involved.

In this opinion piece, the authors outline the prospects for the development of chronic disease vaccines. These might not need to rely on the traditional method of inducing the body to produce antibodies, but rather on introducing monoclonal antibodies against specific proteins — this has so far worked well against Crohn's disease and rheumatoid arthritis.

The authors outline key hurdles in developing a successful therapeutic vaccine. Safety and efficacy are two obvious ones, but there is a third that is unique to vaccines for chronic diseases. Because these vaccines would block bodily chemicals — such as cytokines or hormones — it would not be acceptable for a vaccine to induce a life-long block (unlike a malaria vaccine, for example, where a lifelong block would be ideal).

These might be particularly useful in developing countries, say the authors. Because prophylaxis with vaccines is already a familiar concept, there should be no problem with patients' compliance, and judicious partnerships between public and private organisations could mean the vaccines are produced cheaply.

Achieving the millennium development goals: does mental health play a role?

The Millennium Development Goals (MDGs) practically define health efforts in the 21st century, but they virtually ignore non-communicable diseases such as mental health, say these authors. This is despite evidence that mental health disorders are among the most important cause of sickness and disability and even premature mortality. The authors argue that tackling mental health problems will be vital to achieving the MDGs, and three in particular — eradicating poverty, reducing child mortality, and improving maternal health.

Poverty and hunger are well-recognised risk factors for mental health, but mental health also makes it harder for people to escape the hunger trap. Mothers who are depressed during pregnancy and post natally, are more likely to have underweight babies; not only that, the illness means these mothers are more likely to stop breastfeeding and less likely to ensure their children are properly immunised than mothers without depression.

The authors advocate that strengthening basic health-care systems should be holistic. For example, developing countries need more and better-trained health workers but they should not only know how to deliver babies but also how to counsel new mothers. HIV/AIDS programmes, as another example, should ensure that individuals not only have good access to antiretrovirals but also to treatment for depression if they need it.

What is the best approach to treating schizophrenia in developing countries?

Schizophrenia is relatively rare — affecting 1% of the world's population — but is arguably one of the most severe mental illnesses. Diagnosing and treating it can be hard enough in developed countries; the challenges are magnified in developing nations with inadequate health systems; few trained staff; and pervasive social stigma. So how best to treat it? In this debate, three psychiatrists offer their different viewpoints.

Vikram Patel, at the London School of Hygiene and Tropical Medicine, says the shortage of mental health specialists means that the most effective way of spreading the expertise around might be for non-specialist health workers or community representatives to be trained to bear the brunt of providing first-line mental health services. Saeed Farooq, at Pakistan's Lady Reading Hospital, argues that the principles of the WHO's DOTS TB programme, in which patients are given an uninterrupted supply of medication taken under close supervision, could be used to treat schizophrenia. The rationale is that missing medication for schizophrenia, which can be common given the cognitive impairment associated with the illness, has serious consequences and can lead to much higher risks of relapse. R. Thara, director of the Schizophrenia Research Foundation, Chennai, India, advocates tackling stigma by offering proper treatment. In India at least, he says, the mystification of mental illness is intensified by a lack of awareness about schizophrenia and also by "magico-religious" beliefs. Effective treatment that shows the symptoms to be an illness rather than a religious curse is the best antidote to stigma, he says.
